Predicting the marks required to get a seat in a medical college is quite difficult but when we go through previous year trends then
In neet 2019 , 442 marks for obc category was the cut off marks/ lowest marks that got an mbbs seat in a government medical college under Telangana state quota and it was for GOVT MEDICAL COLLEGE, NALGONDA

But yes,
cut off marks / rank for medical colleges keeps changing every year depending upon various factors like

So , to be on the safer side, so you should try to get at least 500 marks in neet 2021 to get a seat in a Government Medical College

You belong to bc which a state level reservation under obc, so you are eligible for state. But for all India counselling, you will be eligible for obc ncl only if you fall in the central list.
Moreover,
In neet 2021, For all india quota seats, obc ncl reservation was there only in National Institutes/Central
Universities and not in state government colleges.
but as per today's decision now obc ncl reservation will be there in all india quota seats of state government colleges as well.
