As mentioned in the instructions for obtaining the fitness certificate by the JOSAA Counselling, you have to obtain the medical certificate from a Registered Doctor with his Registration Number.

So, if you get your medical certificate form the Registered practitioner and the registration number is mentioned in the certificate, there will be no problem in the counselling process.
