The eligibility for appearing Neet exam Candidate should have passed 12th std exam in science stream,in aggregate of physics chemistry and biology, securing at least 50% marks for the general candidate and 40% for The reserve category candidate.The minimum age to be eligible for Neet examination is just you should have completed your 17 years on December 31st 2021.
Your should have pass your 10+ two level from a recognised board or nios, then only you are eligible for NEET.
Candidates passed 10 + 2 examination along with English are eligible for appearing Neet exam.

It seems that you have asked for NEET UG 2021 because now 2019 had passed and private candidates like if you have studied from NIOS or any other recognized board and passed it with minimum aggregate of 50% along with Physics, Chemistry and Biology as compulsory subjects then you are eligible for NEET 2021. Also note that in your curriculum English subject should also be there.
Also there is no upper age limit for NEET but there is minimum age limit for NEET which is 17 years which means that if you want to appear for NEET 2021 the you must have completed your 17 years on December 31st 2021, if all above eligibility criteria are fulfilled by you then you can appear for NEET UG 2021.

As NEET 2021 applications are not out yet so after it gets released you can apply for NEET 2021 by following the given process :-
> Very first step is to visit https://ntaneet.nic.in/ntaneet/welcome.aspx and fill out the application form, then on first menu you have to do registration using your basic details such as name, father's name, mother's name, contact details and so on.
> Now on second menu item you will have to fill details like your academic performance, address, ID type, medium of question paper and so on.
> Then on Next menu item you have to upload some images such as your passport size photograph (10-200 Kb in JPG format), Signature (4-30 Kb in JPG Format), Class 10th certificate (100-300 Kb in JPG Format), Left-Hand Thumb impression (10-50 Kb in JPG format) and postcard size photograph (50-300 Kb in JPG Format).
> Then you have to proceed for payment of application fee which can be done only in online mode either through Credit/Debit card or UPI or NEFT/RTGS.
> Then after you have to take the print-out of the NEET Acknowledgement receipt.
