1.the class 10 and 12 marks list and certificate

2.category certificate

3.proof id like (aadar card,passport etc)

4.passport size photos

5.neet  admit card and rank card

Neet Admit card,score card,allotment letter, DOB PROOF-10 or 12 th class admit card /marksheet, ID proof-aadhar card,pan card,voter id, domicile of that state if applied through state quota but if applied through AIQ 15% than no domicile required...(check whether domicile is required now or Aadhar card will equally be valid for address proof)...CASTE CERTIFICATE IN PROPER FORMAT(either English or in hindi)... minimum issuing authority is SDO,DM,SDM,TEHSILDAR,ADM,DC etc....,8 passport size photographs

Check the format of your caste certificate (if already issued and match the same with the one provided with the prospectus.(information bulletin neet 2019 -download from ntaneet.nic.in)
