- Mark sheet of Entrance Test (JEE Main / PPT / C-MET / NATA / GATE / GPAT)
- High School 10th Mark sheet
- Date Of Birth
- Educational Qualification Mark-sheet (10th/12th / Diploma / Bachelor’s Degree)
- Local Resident Certificate of Madhya Pradesh, if applicable
- Cast Certificate (SC/ST/OBC/S/FF/TS/HC/NCC/BPL/ClassIV/JK), if applicable
- Passport size photograph
- Candidate’s mobile number and e-mail id
- Aadhaar number
