• High School 10th Mark sheet
• Date Of Birth
• Educational Qualification Mark-sheet (10th/12th / Diploma / Bachelor’s Degree)
• Local Resident Certificate of Madhya Pradesh, if applicable
• Cast Certificate (SC/ST/OBC/S/FF/TS/HC/NCC/BPL/ClassIV/JK), if applicable
• Passport size photograph
• Candidate’s mobile number and e-mail id
• Aadhaar number
