Identity Proof of Beneficiary
Present Address Proof of Beneficiary or Parents in the case of minor
Permanent Address Proof of Beneficiary /Parents in case of minor
Citizen must provide signed copy of Self-Declaration form along with the application form
One Passport size Coloured Photograph of the Applicant
