Hello!
Percentile, in general terms is a statistical measure used in analysing a group of people on the basis of certain parameters.
In case of examinations like NEET, a percentile is used for the purpose of evaluating the marks scored by an aspirant amongst all.
For example, a 90th percentile holder is someone who has scored better than the rest 89% of students and 10% of remaining students have scored better than him/her.
Taking your query into consideration, a 50th percentile means that you have scored better than the 49% students and the rest of 50% are having a score more than yours.
