There is nothing like format in the medical certificate,if you need a medical certificate for your admissions or counselling procedure you can simply contact your family doctor or a doctor nearby your house and ask him whether he can issue a medical certificate for you or not.
That's it.He very well knows what to write in the medical certificate,he will just ask your name andd DOB.
Thus,the doctor has to only show that the particular student is medically fit and fine.
Simple.You just have to pay him the fees for issuing your medical certificate.
