Admission in mbbs in bihar or in any other state is through neet examination only.
To be eligible for neet 2021 one must be either appearing for 12th board examination 2021 or should have passed 12th board examination with physics, chemistry ,biology/biotechnology and English and must have got at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology /biotechnology in 12th examination if belongs to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st category.

15% all india counselling :- If you not domicile of bihar then you can get seat through 15% all india quota on bihar
85 % state quota :- you are eligible only if you domicile of bihar

In neet 2019 , 577 marks for general category was the cut off marks/ lowest marks that got an mbbs seat in a government medical college under Bihar state quota and it was for Government Medical College, Bettiah

But yes,
cut off marks / rank for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the neet examination, kind of marks obtained by students in neet , toughness level of the neet examination, number of seats available etc.
So , you should try to get at least around 50 marks more in comparison of Neet 2019 in your Neet 2021 Examination to be on the safer side, so you should try to get at least around 625 marks.
In case, if you are not domicile of bihar, then you need at least 650 marks for govt medical college in Bihar for all india quota
