Its just a general information form about the candidate appearing for the exam.....like it has name, surnae, father's name , postal address, enclosures about eh mbbs certificate....gender , age .....and all the  bio data......to ensure the eligibility criteria....and the most important among is the certificate of completion of internship....

Like other applications, NEET PG also asks for the general information about you with identity proofs.



Other than that you have to give information about your UG college, year of study, when did you complete your internship, percentage of marks obtained in university exams, etc.
