Going by previous All India cut offs and your rank, you may decent chances in Medical Colleges like:
Kasturba Medical College, Manipal
Jawaharlal Nehru Medical College, Belgaum
KMC, Mangalore
Amrita Institute of Medical Sciences, Kerala
Yenepoya, Mangalore
Govt. College for Clinical branches is tad difficult for this rank. Having said that, this is 2019 data and cut offs change every year depending on various factors such as no.of Candidates appearing for the exam, no.of Candidates qualified, difficulty of the exam, and no.of seats available for each specialisation. I am assuming you are asking about All India counselling. According to previous years data the all India cutoff for MD Pediatrics was AIR 4438. So, there is almost no chance of you being able to get MD Pediatrics under all India quota. Even though the cutoff changes every year it is more or less the same.
You can try and apply in state quota, you may get a seat as your rank is good or you can apply for dnb pediatrics if you are keen on pediatrics.
