Hello there, it seems you are confused between either opting for MBBS or not . If you have decided to become a doctor and serve the community you should go for MBBS. Now if you didn't get college in your 1st attempt then don't worry , if you are determined to do become a medical graduate try it again. If you really work hard you will definitely crack the exam. You should study smart. And if you failed to get in multiple attempts then you should carefully decide whether you want it or not. Now coming to your point about other options then you can go with BDS, or you can choose BHMS, BPTH etc or other paramedical courses. So there are many career options open and many more ways to serve.
