Hello,
when you lock option for NIT warangal then you will get all the formats from the institute. it will also include format for medical certificate.
You have to attest your photo if necessary and get it attested from a certified MBBS doctor.
no need to worry all the format and sample documents will be provided to you after completion of counselling process.
