The form has already been released.

Till now there has been no information regarding the release of application form. It is said that exam form will tentatively release in 3rd or 4th week of June. Exam will be conducted by directorate of higher education, Maharashtra. This exam will be conducted once in a year. So just wait for some time, I think the form will release soon.
