Go to ICAI's Website and click on E-Services , from there select Student services, you will find a list where all details regarding students are given.

If you still can't find the registration form: Please Search "Registration Form for CA Foundation course" in the search section given on the website.

You can also scroll through the FAQ section mentioned on ICAI's website, there would be a question mentioned as to "How to register myself?" They would have provided a link, click on that and it will take you to registration portal/page.
