Hi Madan,
There‘s no separate form for MBBS in ALIGARH MUSLIM UNIVERSITY. Admissions to Mbbs is through All India Central Universities Counselling. You need to register for it through mcc.nic.in and you need to select the option for AMU. As of now, you can register for the mop up round which will begin from 13 August 2019 and payment facility is upto 16 August. Choice filling is on 16 August and result will be declared on 19 August 2019.

For this year the dates were:
- June 19-application release date
- June 26-last date of submission
- June 26-last date to submit fee.
The notifications you can get from Amu website.I am sure you will be seeking this for next year,stay tuned for next application release still then you can prepare better.
