In most cases, when calculating the academic percentage for Class 10 or Class 12 for government exam forms, only marks from the main subjects should be included. But, if the mark sheet does not mention any extra subject then you can include its marks as part of the total percentage. However, if subjects beyond the usual main 5 subjects are specifically marked as additional/optional, then you should not include them.

See in general the marks obtained in 6th subject or extra subject is not included for calculating the percentage in 10th or 12th mark certificate. However if it is not mentioned as extra subject or seperately, then you can definitely use the marks obtained in that particular subject for calculation of overall percentage.
For example in CBSE, music is given as extra subject in seperate column so that should not be taken into consideration while calculating the overall percentage.
