Will I download any medical certificate form from internet nd get filled by govt hospital.. Will it work for admission to nit, iits??

Below are the mentioned certificates that are required :-
There are a few important documents that are mentioned by JoSAA authorities that each student need to carry while reporting at NITs, IIITs or GFTIs. A special mention needs to be made for students that all the documents must be in their original form. No print outs or Xerox shall be accepted.
Heres the list of original documents that a candidate must carry while reporting at NITs, IIITs or GFTIs:
1. Provisional Seat Allotment Letter
2. Original Admit Card of JEE Main
3. One passport size photograph same as on the JEE Main application form.
4. JEE Main Score Card issued by CBSE. (Downloaded once are accepted).
5. Proof of photo ID issued by either central Govt. or State Govt. or last attended School or Admit card of class XII.
6. Medical Report as mentioned on JoSAA website
7. Marks sheet of class X as proof of Name of the Candidate and date of birth.
8. Class XII Board Certificate and Marks sheet of qualifying examination to be used as proof of State code of Eligibility.
a. A passport is required if a candidate clearing the qualifying examination from outside India for for determining the 'State code of eligibility'.
b. The state code of eligibility for Indian nationals passing the Class XII (or equivalent) examination from Nepal/Bhutan will be determined through the permanent address in India as mentioned in the passport of the applicant.
c. To determine the 'State code of eligibility for students having Senior Secondary Certificate, proof of place of examination center of the final examination from the National Open School is to be presented.
9. Category (SC/ST) certificate to be submitted in the format given on official website of JoSAA (Issued by competent authority).
10. OBC-NLC (central list) certificate of category will only be accepted if issued by the competent authority in format prescribed by JoSAA at the official website. The form must clearly contain that the candidate is from Non Creamy Layer.
11. Persons with Disabilities (PwD) according to JoSAA website, must carry certificate issued from a duly constituted Medical Board. For candidates whose disability is temporary, the certificate would be binding for five years. The validity for those who have permanent disability must be mentioned as permanent.
Certificate issued by an invalid Medical Board or expired on the date of reporting will be rejected.

Hi biky,
No any other medical certificates will not be accept by the josaa staff in reporting centres. Annexure 8 will be in the business rules in josaa official website. Here is the link for josaa official website.

You can go through this link and check the business rules and the download the annuxure 8 from that website. Fill that annexure 8 with the doctor and submit to the reporting centres.
