Will my application be rejected if I did not submit a picture with the placard containing my name and the date of the picture taken?

There's nothing to be worried about, even I didn't have my details written under the photo, and no one asked anything about it. They don't usually take a nice look at the admit card, they'll only check the roll number and other important details. It's okay if there are no details written under your picture, but the picture should be there and it should be recently clicked.
