You can only fill the application form of JIPMER by using details given in your Aadhar Card or Passport and will be required to carry either one of these document to the exam hall as ID proof.

So no it won't be rejected if you put your initials.

You should ideally put your official name on the JIPMER application form. The official name is written on your Birth certificate or other identity docuements like class 10 Admit card,Aadhaar card, passport etc. Please do not put initials in the JIPMER application form if it is not included in the official documents.
