The Certificate of Disability is given in NEET-UG Brochure on the Page no.73 Appendix IX, you need to fill this certificate and you can obtain your Certificate of Disability from the following Centres:

1. VMMC & Safdarjung Hospital, Delhi

2. Govt Medical College, Chandigarh

3. SMS Medical College, Jaipur

4. IMS, BHU, Varanasi

5. Grant Medical College, Mumbai

The name of 6 more Centres are given on Page no.74 of NEET Brochure, you can visit any Centre to obtain your Certificate of Disability.

You need to produce this certificate on the Day of Counseling.
