yes you can give civil exam after 4th year orwe can say in final year of MBBS . also you can give the exam after completion of MBBS . however i will suggest you to give it after your MBBS as the civil exams study is not easy as it consumes a lot of time also and it can affect you MBBS too. however its totally upto you , when you want to give

As you did not mention clearly I'm assuming you mean civil service examination.
To appear for civil service examination, a candidate must have attained the age of 21 years and must not have attained the age of 32 years on 1 August of the year of examination.
For OBC the maximum age is 35 years
For SC/ST it is 37 years
So if you fulfill this criteria you can appear for the exam even after your 4 years of MBBS

The essential qualification/eligibility for the civil services examinations are as for general category candidates are:
The minimum age should be 21 and you can appear till 32 years of age. For other category candidates there is some relaxation.
The maximum number of attempts for general category candidates is 6, It varies according to category again.
As per the educational qualifications, the minimum requirement is the completion of an undergraduate degree either from deemed/private/government/distance/open universities.
Specifically for MBBS, the requirement if the internship is not completed is that of providing the necessary proof of documents of eligibility from the from a competent authority at their institute/university at the time of the main examination.
The nationality should be INDIAN and there is some other specific requirements again of documents if belonging to other nationalities or some specific states.
