First of all, you don't need any gap certificate as you have not completed an year after passing your 12th standard. Gap certificate is needed by those students who had passed their 12th standard an year or more than an year ago but due to some reason they couldn't get themselves admitted to the college or resume their studies.
Therefore, you do not need a gap certificate for admission.
You can easily get the the affidavit for gap year certificate online.

Gap affidavit is the self declaration in which it is mentioned that the student has taken gap between the particular period and also it mention the reason for gap. Universities/institutions usually prescribe their own format for a gap affidavit. You must visit the concerned university website for further information. Here is the format that you can use for college admission or for applying to a university.
Format :-
- Name of the candidate.
- Age.
- Residential Address.
- Most recent educational qualification.
- Institute name (from he/she received education).
- Date from which the student has taken gap.
- Reason for taking the gap.
- Name of the College / University where the candidate want to take admission.
- Date of application.
- Signature of candidate.
- Signature of the advocate or lawyer.
As, you didn't took any gap between 12th and bachelor's, there is no need of gap certificate.
