do I have a chance in getting a seat in mbbs college in all India counselling . my rank is 189009 and I got 397 marks. I belong to sc category

congrats for your marks.
you have a good mark which can get you a medical seat. but sorry to say that you will get only a private seat and medical seat in government as the score is high for that.
probably private colleges must be the only choice. though sc category, score for govt is like 500 +
