There's nothing like free seat, even in government medical colleges, you have pay the course fee, it's that fee in government colleges is very low when compared to private medical colleges.

In neet 2019 , 426 marks for obc category was the cut off marks/ lowest marks  that got an mbbs seat in a government medical college under karnataka state quota  and it was for Kodagu Institute of Medical Sciences, MADIKERI

But yes,

cut  off   marks / rank   for medical colleges keeps changing every year  depending upon various factors like  number of students that appeared in the neet examination, kind of marks obtained by students  in neet , toughness level of the neet examination, number of seats available etc.

So ,  you should try to get at least around 50 marks  more in comparison of Neet 2019 in your Neet 2021 Examination to be on the safer side, so you should try to get at least around 475 marks.
