Hello,

You have to apply for it in district magistrate office(if you are residing in city) or else if you have all your relevant documents registered from your native village then you have to file your application in tahasildar's office(highest authority person after HDO in a particular taluka)

This is the list of some important documents which you will require to submit your application.

1)letter of patwari(registrar of a village) that your family belong to that particular village

2)your fourth class leaving certificate

3)your birth certificate

4)your aadhar card

….and many more.

I refer you to have a visit in the respective office and obtain a complete list of the required document to avoid any unnecessary delay by the government officials.
