  2. Download Admit Card and Self Declaration Form : After downloading your admit card, scroll down to find the self-declaration form attached to it.

  3. Read Instructions Carefully : Pay close attention to the instructions to avoid any issues at the exam center. The form includes COVID-19 related information.

  4. Fill in Personal Details : On the first page, enter your name and address accurately.

  5. Report Health Symptoms : If applicable, select options for any major symptoms like fever, cold, or flu. Only mark these if you are experiencing severe symptoms.

  6. Tick the Correct Option : In the section with pointers B, C, and D, make sure to tick option C.

  7. Travel History : For the prompt about recent travel to any city, leave it blank as instructed.

  8. Attach Passport Size Photo and Thumb Impression : Paste your passport-size photo (it should be the same one as on the admit card) and your left-hand thumb impression in the designated areas.

Here, after you download your admit card, on scrolling down you will find a self declaration form attached to it.

Read the instructions carefully as any mistake can land you in trouble on the exam centre. The declaration form consists information about covid -19 so you can fill it easily.

On the very first page you need to fill your name and address. After that you need to select the options if you have had any of the major symptoms mentioned their such as fever, cold, flu etc. Make sure you do not tick any of them untill and unless you have any of them in severe.

Next you will find pointers b, c and d here you have to tick on the c option.

In D it is mentioned if you have travelled to any city in recent times you have to leave it blank.

At last you will have to paste your passport size photo, (The photo should be the exact same which you have pasted on the admit card.) and left hand thumb impression in the respective columns.

You do not have to sign in the column given as it is clearly mentioned that the signature has to be done in the exam centre in front of the invigilator.
