You can get admission in Chattisgarh Medical Colleges under state quota with a score of 478. Following are some colleges

Government Medical College, Rajnandgaon

Late Shri Kashyap Memorial NDMC Government Medical College, Jagdalpur

Government Medical College, Ambikapur

Late Shri Lakhi Ram Agrawal Memorial Government Medical College, Raigarh

You can try for admission in any of the above mentioned colleges as these colleges had a cut off less than 478 for Open Category in 2018. The cut off marks change every year but not in a very wide range, therefore, you have chances of admission in the above mentioned colleges.
