The top 4 percentage of 12th standard boards is determined by the final top 4 marks that you will receive when the results come out.
In case you get better marks in other subjects than either Hindi or English (considering there are 5 subjects) then you cannot add both the subjects.
And if your Hindi and English marks are included in the top 4 after the publication of the results you can very well go ahead with those two to bring out the top 4 percentage.
Although nowadays all the major degree offering colleges ask for your total percentage that is percentage of all the 5 subjects.
If at all, some colleges ask for your top 4 percentage, make sure to give them the best marks in the 4 subjects that you scored.
I personally had Hindi and English too in my 12th standard and I never faced any problem in adding marks of both the subjects when required.
