You shouldn't add salutation in front of your name, father's name and mother's name in NEET UG application form. However, you may not face issue while attending the exam so you do not have to worry about that. As long as the name of your father is correct your candidature will not be rejected.

In place of fathers name, usually the salutation is given automatically. But as you have made some extra effort and put the salutation then in general it can come out as Mr. Mr. <your fathers name>. But you have nothing to worry about it. You may have change the details during the correction session but as you haven't done it, there will be no such big problem in the exam center. It may cause some issues during counselling and document verification but at that moment also such cases are considering with good measures and you will suddenly not be cancelled your admission, instead you would be dictated about some of the rules.

Don't worry about it and just focus on your revision.
