Dear Dinesh, not mentioning name and date on the photograph will not lead to rejection of your application. Provided rest all details have been furnished correct and the uploaded photograph, thumb impression and signature follow the given guidelines, there should not be any reason to reject your application. So do not worry about it. Usually NBE notifies Candidates in case of any discrepancy of the uploaded photo and signature. Please wait to hear from them. If you do not receive any notification, consider your photograph to be accepted. If you want, you can reach out to the helpdesk at: neetpg@nbe.edu.in but that would be totally unnecessary.
Its mandatory to caption photo with name and date, I had called NBE helpdesk regarding the same.

there is no mention of name should be in the phograph of neet mds application form. so you dont need to worry.
