I passed 12th exam in 2018 but reappearing in 2019 as private for improv % for jee what should I fill in school name ,address year of apper

When one is reappearing for the 12th for improving their score, they are not affiliated with any school. Thus, one can give the school's name where one initially did their 12th standard and its address. As for the year of appearing, since you are appearing in 2019, fill the year as 2019.
