See considering BC-D under OBC category as per the reservation criteria of Telangana state, you will get seat in the Telangana as per the reservation benefits of OBC category.

As we go through previous year's (2019) cut-off then it shows that in 2019 the lowest cut-off marks was 504 for OSMANIA MEDICAL COLLEGE, HYDERABAD  under state quota for OBC category and we saw an increment of around 25-30 marks in institutional cut-offs in 2020. So if you are targeting for NEET 2021 then your aim should not be less than 550 out of 720 marks especially for Government medical colleges in Telangana and for OSMANIA MEDICAL COLLEGE, HYDERABAD as well.
