Is annexure 'A' compulsory to provide with ews certificate ,if it is then whose signature is required on it

Annexure-A is just for only reference purpose so that candidate can know what are the eligibilty criteria should a candidate possess to avail the benefit of EWS reservation.
Though, Annexure-A contain the prescribed format(page no. 7 "Annexure-I") of EWS certificate which is made compulsary to have it only in given format and only then it will be applicable and accepted as a proof for claiming as EWS category.
For signature purpose, refer to point 5 at page no.2 of Annexure-A.

I don't think I can get that signed in time, but I have filled ews category,how can I change it back to general cuz I can get ucms thru general
