The only three MBBS entrance examinations which are legally allowed in India right now are NEET, AIIMS and JIPMER

so apart from the AIIMS and jipmer forms which are also filled ,online you do not need to fill up any other form except for the Neet which serves as the main medical UG entrance examination for admission to MBBS courses across the different medical colleges across India weather be fit through the 15% all India quota or 85% state quota

you take my advice, prepare well for the Neet examination and it is your best opportunity to crack a medical seat in a college of your choice in India.

The only entrance examination for all the MBBS seats in all the states now is NEET. So you don't need to fill up any other form except NEET.

The other two examinations are held separately for the two prestigious autonomous institutions, namely AIIMS and JIPMER. So you need to apply for these two examinations separately.
