i think proforma is not needed as the information bulletin is not updated......it the same (carbon copy)  as 2019 neet information bulletin. take only what the hall ticket instructions say.

The admit card that is available online is a 3 page pdf. The first page is the Admit card and the self declaration form. And the next 2 pages are the Advisory for Candidates from NTA. You have to downlaod this 3 page pdf. This is the proforma. You will have to fix one passport size photograph on the admit card, one on the self declaration form. You will have to carry one more passport size photograph for attendance sheet. To be on a safe side, we would suggest you to carry 2 passport size photographs. Also this time, you can carry Masks and Gloves, 50 ml Sanitiser and a transparent water bottle along with your Admit Card, Valid Id proof, PWD certificate if applicable and your passport size photographs.
