The format of the certificate is given in the JoSAA website. Take a copy of it to any government hospital and get it signed by the CMO or SMO of the hospital.

Hey there, for getting that certificate you need to see the IIT or NIT website and it might be given there as mainly it is being provided by the colleges only and you can even ask at the helpline number of that.

Otherwise, you can take medical certificate from the government doctor in your city mentioning your weight, height, any disease, and mentioning some diseases too and asking about your specs whether there or not like that details.

Every year JOSAA releases the format of various documents which includes the format of the medical certificate also. You have to download the business rule file from JOSAA 2019 official website and find certain annexure which contains the format of JOSAA'19 medical certificate.
