Which state to be filled in column for State of Eligibility for 15% quota of seats? Is it your domicile state or state from where you are appearing for your 10+2…?

It will be the State you hold the domicile of. This is because you will be eligible for the rest of the States barring Jammu and Kashmir and Ladakh, and you cannot all those State's details, you will have to select the State you hold the domicile of to participate in 85% State Quota.
