See the first thing you need to understand that there were very few questions which were actually out of NCERT- by that I mean that there was nothing mentioned about those topics in the NCERT by itself. Most of questions which were touted as ‘out of NCERT’ were actually application based questions from topics that were already mentioned in the NCERT.
86–87 of the questions are usually either lines picked up directly from the NCERT itself or are application based questions. The rest 3–4 questions are out of the NCERT. So if you really need to secure a 100% score atleast in biology, the NCERT all by itself won’t suffice- rather you must also follow some standard reference books ( or your study materials if you attend any coaching classes) accompanied by practice and regular assessment of your preparation.

Most of the questions are asked in NCERT for NEET but some question is not. They include in your syllabus. This type of questions are not included in ncert books. So, you should cover complete syllabus also. You can check the preparation guidance from toppers through NEET Toppers.

If at all some questions look like they are not at all related to the textbooks, you don't need to worry about those questions as they will be new for everyone and thus will not be the rank deciding questions.
You should know how to differentiate between must know, should know and good to know concepts.

Neet is an all India entrance exam for students who want to opt for medical and paramedical courses. So most of the questions are application based to exclude the students who are not strong with basic concept. To be able to solve such question, please try to comprehend the logic behind the theory. Solve many mcqs. Be very strong with your concepts. It's most important in physics n chemistry where your concepts are tested. Many questions in physics n chemistry are from out of the book, application based. So more mcqs you solve, better is your chance of getting these questions right on exam day
For bio do read ncert textbook, that is more than enough. Also solve mcqs for biology.

NCERT textbooks are the basis on which NEET questions are formed. This is because they are the pro version of topics we study in MBBS.

Sometimes questions are definitely out of NCERT but there is a particular trend in those questions so can be tackled easily if a list of those topics are made and read from you notes or coaching modules . Why they are asked? Cause they are looking for future doctors and it’s a huge responsibility so they check it if students can think out of the book too or not . Probably
We can't say that it is completely out of NCERT. But the questions may be related to the concepts/chapters in NCERT or the practical application of the concepts in NCERT.
It may appear out of syllabus but it will be related to one way or the other. It is usually asked to assess the candidates and to set the difficulty level of the question paper.
See if the NEET examination was so far completely ncert based than the score would become like 720/720 for the first ranks so to maintain the standard of the examination few complex but not beyond the syllabus the questions are asked so as to make the candidate check his mental ability listen those questions are not beyond the concepts of ncert they are related but bit confusing see such questions comprises 20%of the paper rest all 80%are direct once.
