Yes of course you can apply for B-category/Management quota seat irrespective of your marks because the seats are allotted on the basis of merit and also note that if you talking about state quota counselling of AP then you have to follow the schedule which has been released by Dr. NTR University of Health Sciences, Vijayawada for different rank groups.
On such marks you have good chances of getting some medical colleges in Andhra Pradesh under state quota according to previous year's cut-off. As we go through previous year's cut-off then it shows that you can get following colleges :-

As you already contacted the KEA help line center regrading this problem and got a response to send the document through mail, maybe they will definitely consider your document for the procedure. Maybe should again call the KEA help line center and follow up the details. So that they can be updated about your problem.
Hello, since KMC Mangalore and Manipal are Deemed University and you are other state student, you will have to apply through All India Counselling itself and there are no management quota seats.
However, in MS Ramaiah and St Johns, fees under management quota will be anywhere between 5-20 lakhs per year. Fees is collected annually and not as 5.5 years

If you are willing to get your admission done in St. John's Medical College, Bangalore then, you must know about the last year's cut off. The cut off score for NEET 2019 in St. John's Medical College of private seat was 528. Since you have scored 582, it seems that you have a fair chance to get a Private seat in this college. Apply for the State Quota Counselling of Karnataka to secure a seat.

According to NTA rules, if you get a college in the first allotment, you can still sit for the second counseling. But if and when you get admission or a seat allotted in the second counseling, the seat in your first college is automatically canceled. Although you get the documents back, the fee that you pay for the counseling is NOT returned.

St. John's is a Private Medical College. Private Medical Colleges do not participate in All India Counselling and the seats are filled through State Counselling alone. And for appearing for State Counselling you need to hold the domicile of that particular State. Or else the only other option is through Management Quota. So if you belong to Telangana, you should participate in Karnataka's State Counselling for admission to St. John's.
