HSC Mark Sheets-cum Provisional Certificate (or equivalent Board). If an applicant has not received the mark sheet from the Board in-time, then the download mark sheet from Internet will be admissible.
Conduct Certificate from the institution last attended.
Caste Certificates from revenue office not below the rank of a Tahasildar.
Schedule Tribes Certificates from revenue office not below the rank of a Tahasildar.
PH/OH Certificate from CDMO /SDMO/CMO of Capital Hospital (in case of Bhubaneswar)
Ex-Serviceman Certificate from Zilla Sainik Board.
Certified true copy of NCC Certincate from the competent authority.
Certified true copy of the order of transfer & joining in the present station.
Certified true copy of Migration Certificate for the applicant from other Boards except BSE, Orissa.
Certified true copy of permanent resident/nativity certificate (for OSA & OLNS candidates)

Note: There is 16.25%, 22.5%, 5% and 1% sanctioned seats reserved for the SC, ST, PWD and Ex-Serviceman, Serving Defense Personnel and Children of Martyrs.
